INFORMATION
REGISTERING YOUR PET
Appointments are available on a first-come, first-served basis. Please review the following information that will be required to register your pet: I understand the charges explained to me and I agree to the sterilization surgery, rabies vaccine, age-appropriate vaccinations, and microchip performed by El Paso Animal Services as stated. City Registrations will not be provided at this event and can be obtained at my personal vet or at El Paso Animal Services the week following my pet's surgery. I expect Animal Services to use all reasonable precautions against injury, escape, or death of my pet but the City of El Paso and its representatives will not be held liable or responsible in any manner and I assume all risks. I shall fully indemnify and hold harmless the veterinarian, Animal Services, and the City Of El Paso from any and all lawsuits, claims, demands, and liability. I am aware that the clinic is not a 24-hour care or emergency facility. I have been advised not picking up my pet between 2 pm - 4 pm means they will be taken to the shelter where there is increased risk of disease. I understand anesthesia and surgery always carry with them the risks of infection or death which are increased if my pet is ill or not vaccinated. I understand it is recommended routine bloodwork and up-to-date vaccinations are completed at my primary veterinary clinic prior to surgery to reduce that risk. I have been provided the pre-surgical and post-surgical instructions and understand if they are not diligently followed, complication may arise. I am aware that any post-surgical complications that may occur will need to be resolved with my primary care veterinary clinic, and I understand I will incur all associated costs. I understand that if my pet is pregnant, that the pregnancy will be terminated and my pet will be spayed. By submitting this appointment request, I understand that there will be no refunds or rescheduling of appointments provided once my pet's spay/neuter appointment is confirmed. I understand that my $25 payment is a deposit, and I will be given the option to have it refunded to me or donate back to Friends of EPAS once my pet's surgery is completed. I understand that if I don't show up for my pet's spay/neuter appointment or cancel within 72 hours that my $25 deposit will not be refunded back to me. I understand that I will be given pre-op and post-op instructions, and following these instructions will reduce surgery complications. I understand that this is a one-weekend event and I cannot reschedule my appointment.

FREQUENTLY ASKED QUESTIONS
Question: There are no appointments for my pet's size, how can I get an appointment?
Answer: Spots are limited, so if there are no more appointments for your pet's size, that means all appointments for that slot are booked.

Question: If all the slots for my pet's size or sex, can I sign up for a different one?
Answer: No, if slots for your pet's size or sex are already filled, that means there are no more appointments available for that size or sex. If you schedule your pet for a different size or sex than what your pet is, your appointment will be canceled.

Question: What if my pet has underlying health conditions? (diabetes, heart murmur, seizures, hypothyroid, hyperthyroid, etc.)?
